We request that you fill out new patient forms and provide a copy of your insurance and identification card BEFORE your appointment time. (For example, if your appointment is scheduled for 2pm, paperwork should be completed before 1:55pm.) You may download new patient forms at HERE or arrive AT LEAST 30 minutes before your appointment time to complete these forms in the office. Failure to complete forms or provide valid insurance information before your scheduled appointment will require your appointment to be cancelled. 

You may reschedule your initial visit once. If you "no call, no show" your initial visit, we will not be able to reschedule you. 

Reminders for your appointment will be automatically texted 48 hours before and also 2 hours before your appointment. Replying anything other than YES or NO to reminder texts cannot be seen by our office and will result in an "unconfirmed" status in our computer system.

Insurance information must be provided before your appointment. Any accepted HMO plans require a referral (and authorization number) from your primary care provider and must be obtained before your appointment.

Our office is open Monday through Thursday from 8:30 am to 4:30 pm, closed from 12 tp 1 for lunch. No one is available to answer phones, texts, or emails on Friday, Saturday, Sunday or most major holidays. If you experience a mental health crisis, please call 911 or visit your nearest emergency department.
